DATA SHARING CONSENT FOR ACOP THROUGH FACIAL AUTHENTICATION WITH LIVENESS CHECK
  1. I hereby authorize and give my explicit consent to Social Security System (SSS) to collect, use, share, store, and process my personal information and records, such as full name and date of birth, including any sensitive personal information, for purposes of my compliance with the Annual Confirmation of Pensioners (ACOP) Program.


  2. I hereby give my voluntary, explicit, and informed consent to SSS to collect, use, and share my personal information, such as full name and date of birth, with the Philippine Statistics Authority (PSA) through the Philippine Identification system (PhilSys) for verification and authentication of my identity in compliance with the ACOP Program.


  3. I hereby give my voluntary, explicit, and informed consent to PhilSys to share my personal information, such as full name, date of birth and photo to SSS, including my PhilSys Number (PSN) or PhilSys Card Number (PCN), as well as other necessary demographic or biometric information, for verification and authentication of my identity in compliance with the ACOP Program.


  4. I acknowledge and trust that SSS and PSA shall maintain the confidentiality and security of all personal information provided, employing appropriate organizational, physical and technical measures, in accordance with Section 24(c) of the Social Security Act of 2018 (R.A. No. 11199), PhilSys Act (R.A. No. 11055) and the Data Privacy Act of 2012 (R.A. No. 10173). I understand that SSS and PSA shall not disclose my personal information to any third party unless authorized by me or required by law through a valid subpoena or court order.
